When you are done viewing all 3 videos, click on the button below to fill out the "Viewing Affidavit Form".  Once submitted, your certification and CE hours will be uploaded to State-Based Systems within 3 business days and you will receive an email confirming that has been done along with a Stop Loss Certification certificate.
